Sports physicals for all Streetsboro High School and Middle School student-athletes are being offered by Robinson Memorial Hospital May 17 for a cost of $25.
These physicals will be good for the 2008-2009 school year.
The physical exam form can be picked up at the middle school or high school office prior to the exam and completed by a parent.
They also may be found at www.rockets.sparcc.org under parent forms/sports physicals.
Physicals will be by appointment only.
